Connor Kemp is the Performance and Evaluation Lead for the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT). Connor brings to his role both personal experience navigating the health‑care system and a strong commitment to data‑driven improvement. Connor supports the evaluation of the Health Home mission, with a focus on assessing how new models improve access, experience, and outcomes across the Quintuple Aim for people with diverse health needs.
Suzanne Pashley is a Data Analyst for the FLA OHT. Suzanne brings experience from her previous role at Public Health Ontario, where she analyzed health data to support population health assessments and reporting. She is passionate about the use of data to drive positive change in health care and to support equitable and accessible care. In her role with the FLA OHT, Suzanne focuses on analyzing data to inform initiatives that enhance the quality and accessibility of care for the diverse populations of Frontenac, Lennox & Addington counties, in alignment with the organization’s people‑centred Health Home mission.
