These Word forms are designed to support instructors, students, and academic integrity leads throughout each step of an academic integrity investigation or appeal. They are tailored to meet the requirements of Queen’s Academic Integrity Procedures and help users provide the appropriate level of detail.

Initial form required for all instructor-led investigations

Form required for all appeals within your faculty or school
If you are appealing to the University Student Appeal Board, please refer to “Starting an Appeal to USAB” section on their webpage for their required form.
Form Features
- Use of purple text to indicate where information should be entered. It disappears as the user types, replaced by their response in black.
- Embedded guidance and examples to help users understand what to write in different scenarios.
- Pull-down menus to customize the form for each Faculty or School.
- References and links to specific sections of the Academic Integrity Procedures for those who want to better understand why they are being asked to provide certain information and/or how that information may be used.

Student-Friendly Design
- Use of summary blocks and FAQ-style headings to help students quickly navigate the forms.
- Conversational “next step” sections (e.g., preparing for a meeting) to make the process less intimidating
